Hello Everyone.

Here is the link for the Health Assessment we use frequently at Organic MD. It is simple to do and we recommend you to fill it out right before a change in your diet or routine and then maybe a month after the changes.

So if you are planning to go on a diet, or do a fast, fill it out and check your score. Then put it away. After a month do it again and see what if anything changed. Is your score different? Better? Worse? The same?
