Part 3 of 10 - Approach to treatment of Chronic Lyme Disease
The divergence between conventional and integrative medicine is nowhere more contentious than in the approach to chronic Lyme disease—often clinically referred to as Post-Treatment Lyme Disease Syndrome (PTLDS) by conventional physicians or Chronic Lyme Disease (CLD) by integrative and "Lyme-literate" physicians (ILADS).
Because Borrelia burgdorferi (the Lyme spirochete) is highly adept at evading both the immune system and standard testing, the two fields operate under very opposing treatment models.
1. Pathophysiological View of the Disease
- Conventional Approach: Operates on the premise that a standard 10-to-28-day course of antibiotics eradicates the active bacterial infection. The conventional approach views persistent symptoms (fatigue, joint pain, brain fog) as an autoimmune or post-infectious inflammatory sequel. The active infection is gone; the body is simply suffering from residual structural damage or an overactive immune response.
- Integrative Approach: Operates on the premise of persistent, low-grade infection alongside a web of complicating factors. Integrative physicians argue the bacteria can survive standard treatment by forming biofilms, shifting into cystic forms, or hiding in deep tissues. They view the issue as a systemic collapse compounded by untreated tick-borne coinfections (e.g., Bartonella, Babesia), other co-existant infections (usually viral, as well as dental, parasitic and dysbiotic), mitochondrial damage, and profound immune dysfunction.
2. Diagnostic Testing & Interpretation
- Conventional Approach: Strictly relies on CDC-standardized, two-tier serological testing (ELISA followed by a Western Blot). Doctors attribute a patient's persistent symptoms to other conditions like fibromyalgia or chronic fatigue syndrome if their Western Blot does not meet specific band criteria.
- Integrative Approach: Views standard serological testing as highly insensitive for chronic cases, noting that a suppressed immune system often cannot produce measurable antibodies. They use history, clinical symptom checklists, history of exposure, and advanced functional laboratories (e.g., EliSpot, immunoblots, and specialized PCR testing). A negative standard test does not rule out the disease in an integrative framework.
3. Primary Treatment Strategies
Conventional Approach (Symptomatic & Guidelines-Based):
- Antibiotics: The guidelines strongly discourage continuing long-term or repeated courses of antibiotics after completing the initial standard course, citing clinical trials that show no long-term benefit and significant risks.
- Management: Treatment shifts strictly to managing symptoms. This includes NSAIDs or biologics for joint pain, gabapentin or pregabalin for neuropathy, sleep aids, and antidepressants or cognitive behavioral therapy to help cope with the stress of a chronic illness.
Integrative Approach (Eradication & Systemic Support):
- Herbal Protocols: Integrates potent, clinically studied evidence based herbal antimicrobials known to disrupt biofilms and kill persistent forms of the spirochete. Working with your clinician to determine the best protocol gives quicker and better results.
- Biofilm Dissolvers: Uses specific enzymes (like serrapeptase, lumbrokinase, nattokinase etc. or stevia extract) designed to break open the protective biofilm shielding the bacteria from antimicrobials and the body’s immune response.
- Detoxification Support: Prioritizes managing the "Herxheimer reaction" (a severe systemic flare caused by endotoxins released when the bacteria die). This includes prescribing binders (like cholestyramine or activated charcoal), glutathione, and encouraging infrared saunas or lymphatic drainage to help the liver and kidneys process cellular debris.
- Systemic Detoxification Protocols: If there is evidence of a large total-body load of toxins, such as heavy metals, agricultural chemicals, sulfites, solvents or mycotoxins, there is a need to lessen this load. Toxins, like co-infections, weaken the body’s immune response and delay healing.
- Layered Antimicrobials: (Last resort for many Integrative Doctors). Uses pulsed, rotating, or long-term combinations of oral, intramuscular, or intravenous antibiotics to target different lifecycle forms of the bacteria and address hidden coinfections.
4. Immune and Cellular Rehabilitation
- Conventional Approach: Rarely addresses cellular or mitochondrial mechanics directly, relying instead on systemic rest and gradual physical pacing.
- Integrative Approach: Recognizes that chronic infection decimates cellular energy production. They aggressively support mitochondrial repair (using CoQ10, alpha-lipoic acid, and NAD+ precursors), use Low-Dose Naltrexone (LDN) to calm neuroinflammation, and address gut barrier integrity, which is frequently damaged by the heavy antimicrobial burden required to fight the disease.
